CONSENT TO THE PROCESSING OF PERSONAL DATA Legislative Decree n. 196 30Th June 2003 amended with Legislative Decree 101/2018 European Regulation on the Protection of Personal Data n. 2016/679- GDPR (General Data Protection Regulation). The provision of personal data is optional, but any failure to consent to their processing will make it impossible to participate in the activities connected to VIGNALE IN DANCE event. I, the undersigned, DECLARE that I have received all the information referred to in the European Regulation 2016/679, art. 13, in particular the rights recognized by this Regulation and to consent, pursuant to and for the purposes of the art. 7 et seq of the Regulation, to the processing data of my son/daughter, including particular ones, in the manner and for the purposes indicated in the information sheet itself, however strictly connected and due to the management of the purposes referred to point 2 of the information.

Images Consent

IMAGES CONSENT I agree to give free permission to the organization too use the own name, reproduce and fix by ay process and /or to transmit, disseminate the recording and/or pictures of my image, which will be taken during the event, for ads, promotional and commercial purposes, such as publications in newspapers, magazines, the use of videos, films and photos satellite and cable tv networks, in internet, through the social media or web tv.

Medical Certificate

MEDICAL CERTIFICATE The undersigned or the minor has undergone a medical examination in the last 12 months, being declare physically ok for non- competitive activities and being in possession of a medical certification of good health exempting the origination from any civil or penal responsibilities, related to any physical, moral, biological and any other possible damages that may occur during the performing activities in the hosting structures.

Lost objects

LOST OBJECTS I declare to exempt the organisation from any reimbursement obligation in the event of loss, theft or deterioration of things, personal effects of the participants brought in the hosting structures .
